This website is a design prototype. Its appointment form is for demonstration only. Entered form details are not transmitted or stored by this prototype. Please use sample information and avoid entering medical or sensitive information.

The prototype uses self-hosted fonts and images. It does not add analytics, advertising scripts, tracking cookies or browser storage. The local web server may record ordinary page requests in its access log.

Before booking opens

A complete, clinic-approved privacy policy will be published before any live appointment service is enabled. It will explain the responsible organisation, what data is collected, the purposes of processing, retention, recipients and how to make a privacy enquiry.

This notice is a placeholder for the prototype, not the final clinic privacy policy.
